Accomplished programs include counseling, addiction and mental health evaluations, education and training, consultant for program development, community outreach, participation in grant writing and transitional/sober housing.

Skye obtained her bachelors in Science from University of Utah where she continued on to receive her Masters in Medical science from Yale University. She is a nationally certified PA-C with an emphasis in Primary Care. In her spare time she enjoys to spend time with her husband, kid(s), and dog. Some hobbies include traveling, going to the beach, interior design, salsa/bachata dancing, and listening true crime podcasts.


Ebony R Davis

Brief profile (Designation) - Ebony Davis LCSW-QS, MCAP, CBHCMS, is currently pursuing her Doctorate degree in Social Work and was awarded her Masters degree in Social Work from Florida State University and is a Florida Licensed Clinical Social Worker and Qualified Supervisor, Masters Certified Addiction Professional, and a Community Behavioral Health Case Manager Supervisor. She has worked in a number of treatment settings, including substance abuse treatment centers, both inpatient and outpatient, mental health facilities, hospitals, community service organizations with clients from the bottom of the barrel to the boardroom, from adolescents to seniors. Her various roles have included individual and group therapy, assessment, treatment planning, crisis intervention, and case management. Ebony has been practicing social work for over 20 years.

"Prior to being a Nurse Practitioner I have been a nurse for many years in a hospital setting treating patients in medical/surgical area specially in orthopedics and neurosurgery. As a NP it has been my pleasure to see patients for primary care, opiate dependence, pain management in a clinical setting as well as other settings such as group homes and other assisted living facilities. I overheard a surgeon once saying to a new staff that if you treat the patients like they are your grandma/grandpa or other family members then you will do well in healthcare. I truly believe in what he said and treating patients with empathy, respect as well as the best medical care I can provide is of utmost importance to me. "
